1. You will need to schedule a consultation using the booking link (see appointments tab). The first consultations must be an in-Person at our West Linn clinic. Follow up visits can be virtual.

2. Complete all the necessary paperwork PRIOR to the consultation. You will receive a portal log-in to access paperwork once you make your appointment. If you are having trouble with this - you can come early to your appointment to fill it out. 

3. Lab work is required within the past 90 days, which needs to include these labs:

- Complete Blood Count (CBC)

- Comprehensive Metabolic Panel (CMP)

- Thyroid Stimulating Hormone (TSH)

- Hemoglobin A1c (HbA1c)

- Lipid Panel

- Vitamin B12

4. Once the consultation is completed, and if you qualify, you can choose to be sent home with the medication, or have injections done for you in clinic, whichever is most comfortable to you. You must follow up every 4 weeks to continue receiving your prescription and medications.

1. $75 deposit when booking the appointment. If you are a candidate for further treatment, this applies to your first month's fee. If you are not, this is non-refundable, and it will be applied to your patient's wallet for future services at Lume Med Spa.

2. $550 monthly, or prepay $1500 for 3 months 

This includes your initial consultation: a 60-minute consultation with Brooke Sullivan, FNP-C, to discuss your health goals and create a personalized treatment plan. This visit includes an in-depth health history, review of recent or ordered lab work, and education on how GLP-1 medications (such as semaglutide or tirzepatide) work. Injection technique, dosing schedules, potential side effects, lifestyle recommendations, and safety considerations will be reviewed. Time is also dedicated to answering questions and ensuring you feel confident and supported before starting treatment throughout your weight loss journey.  Weekly 15-minute injection appointments OR Monthly 30-minute check-ins. You must follow up at least every 4 weeks for a renewed prescription. 

If you discontinue your treatment before a 3-month period for any reason, your remaining pre-payment will be placed in your client wallet for use on other treatments offered at Lume Med Spa; a refund will not be given.
